Housing Coordinator

The HSS Housing Coordinator manages a caseload of 7-9 clients independently, documenting visit notes daily and addressing barriers to housing access. They conduct housing assessments, develop support plans, and establish relationships with landlords. Regular in-home and community visits are conducted to evaluate client progress. Collaboration with the management team is essential, along with expanding knowledge of local resources and assisting clients in applying for benefits. Maintaining professional communication and adhering to privacy practices is crucial.

**Requirements include 1-3 years of human services experience., valid driver's license/reliable transportation and ability to transport clients.
