We do not share, sell, or use client information for any purpose outside of fulfilling the specific consultation request submitted. Your clients remain yours — we do not “poach” referrals or misuse submitted data in any way. Our mission is to support providers and clients with integrity, transparency, and professionalism.
📝 Our Consultation Process
When you refer a client for an HSS Housing-Focused Person-Centered Plan (HFPCP) consultation, here’s what you can expect:

✅ Step 1: Review & Consultation

We review the submitted referral form and documentation.

Our team conducts the HFPCP consultation remotely with the client.

✅ Step 2: Plan Finalization

Within 1–2 business days, we finalize the HFPCP.

We send a signature request to the referring HSS provider (who will serve as the Transitioning/Sustaining Provider on the HFPCP).

✅ Step 3: Submission for Eligibility

Once we receive the signed HFPCP, we submit it within 24 hours to DHS for eligibility processing.
